Upon his recent retirement, Gastroenterologist Dr. Henry Kung nominated Dr. Zheng's office as the official custodian of medical records of all his patients. Transfer of Care
Dr. Zheng is accepting transfer of care from all patients of Dr. Kung.
Priority will be patients due for colonoscopy /colon cancer screening, upper endoscopy for Barrett's esophagus, patients who are currently ill, and patients with chronic GI issues needing ongoing care.
We do encourage all patients to contact us to schedule a meet and greet appointment with Dr. Zheng, even if you are not currently due for procedure. This will help us establish a great patient doctor relationship before potential urgent needs arise.
It's anticipated that there may be an increase of demand next 1-2 years from postponing cancer screening during COVID pandemic. An introductory appointment will help you secure an appointment ahead of time before your procedure is due.

We have openings for weekend and night clinics to accommodate everyone.
Communication
Dr. Zheng is in the process of delivering welcome letters and/or calls to patients of Dr. Kung.
Like Dr. Kung, Dr. Zheng is sending you remind letters and emails when you are due for colonoscopy /colon cancer screening, upper endoscopy for Barrett's esophagus, screening of high risk patients for stomach cancer, etc. We will also try to have office staff call to follow up and make sure reminds were received during transition of care.

Your Medication Refills
For non critical medications, such as acid reflux or constipation medications, the best way to refill your medication is to email us at: Staff@YiZhengMD.com
Please include your name, date of birth, address, name and dosage of medication, pharmacy and insurance information.

If you are taking medications for more serious medical conditions, such as IBD (ulcerative colitis, Crohn's disease, microscopic colitis, hepatitis, cirrhosis, liver transplant, etc). Dr. Zheng can provide one urgent refill. It's best to schedule an appointment with us first so Dr. Zheng can monitor you.

Your Medical Records
If a record is needed for a medical emergency, Please provide us with the info of the MD that your are getting care from, Dr. Zheng will communicate with your doctor directly.
As official custodian of medical records of Dr. Kung's patients, we not only look after your health care information, but also provide timely remind letters/calls when patients are due for colonoscopy, endoscopy Barrett's esophagus screening and stomach cancer screening for increased risk patients etc. You don't need to contact us just to check your due date.

If you need a copy of your medical records for reasons that's not stated above, please email us at Staff@YiZhengMD.com. Please include your name, date of birth and email address. Please kindly allow 8 weeks for non medically urgent requests.
In additional to safeguard your medical record, Dr. Zheng's office is also in the process of transferring essential part of Dr. Kung's paper charts to electronic medical record.

Priority will be patients who have transferred care to Dr. Zheng and who will be seeing Dr. Zheng soon, as well as patients due for colonoscopy /colon cancer screening, upper endoscopy for Barrett's esophagus and/or stomach cancer screening etc.

Even after transition of electronic medical record, all paper medical charts will be kept according to medical legal requirements.
